The aim is to successfully infect and kill every single human using your disease, before it is cured. If you do that, it'll tell you that you won.

But I've done that. Every country was infected it said. When it shows the world it shows total number dead and 0 infected, every country has fallen.

You have to make sure every person is infected, the game will alert you when that is true. What you did was infected every country and went lethal killing before "everyone" in every country was infected, because you killed people faster then you could spread the virus.

That logic doesn't follow to me, how can it kill people it hasn't infected?

It can't. That's the point. You make the disease as infective as possible, ideally with minimal/no symptoms so it goes by unnoticed. Once a sufficient number of people have the disease, you mutate it so that it becomes lethal. If you make it too lethal too early, you'll kill everyone with the disease before it has a chance to spread to everyone in the world. Timing the mutations appropriately is the key to killing every single human and, thus, winning the game.
